Photo Release for Minors Statement

I, the parent/guardian of the child(ren) above, give my consent for my child(ren) to be photographed for print, video, or electronic imaging (for use on the parish website). I understand that the image may be used in promotional materials, news releases, and other published formats by the St. Paul/Christ the King Religious Education/Confirmation Program. I acknowledge that the images will be the sole property of the Catholic Parishes of Athens & Meigs Counties.
